A Plus Healthcare at Home is seeking a full-time Physical Therapist to provide in-home visits to our patients in Utah County.

Responsibilities include:
- Provide a comprehensive patient evaluation and develop an appropriate plan of care within home health regulations
- Perform physical therapy visits within the PT scope of practice that include training and education of patients and their family members as needed
- Perform timely and appropriate documentation
- Provide oversight and supervision of Physical Therapist Assistants, and supervision of Home Health Aides
- Communicate with other members of the care team, and participate in patient care conferences
Qualifications:
- Graduate of an accredited Physical Therapy program or university
- Valid professional PT license for the state of UT
- Home health experience preferred, but not necessary
- Current driver’s license in good standing
- Reliable transportation and auto liability insurance.
