Hospice RN Case Manager

Forest Park Hospice is seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will be responsible for managing the care of hospice patients and their families in the Portland Metro area. The ideal candidate will possess strong clinical skills, excellent communication and interpersonal skills, and a passion for providing high-quality patient care.

Key Responsibilities:

  • Conducting patient assessments and developing individualized care plans
  • Providing symptom management and pain control
  • Educating patients and families about hospice care and end-of-life issues
  • Coordinating care with other healthcare professionals, including physicians, social workers, and spiritual care providers
  • Monitoring and documenting patient progress and making appropriate adjustments to the care plan
  • Providing emotional and spiritual support to patients and families

Qualifications:

  • Current RN license in the state of Oregon
  • Excellent clinical skills and knowledge of hospice care and end-of-life issues
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Comfortable working with patients and families from diverse cultural backgrounds

Job Type: Full-time

Pay: $49.00 – $55.00 per hour

Expected hours: 40 per week
