We are seeking a Hospice Home Health Aide – CNA to join our team in Portland, OR. In this role, you will be responsible for providing compassionate care to our patients in their homes. You will work closely with our interdisciplinary team to ensure that our patients receive the highest quality of care. Your primary goal will be to help our patients maintain their independence and dignity while providing them with the support they need.

Minimum Qualifications:
- Active CNA certification.
- Minimum of 1 year of experience in home health or hospice care.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.
Preferred Qualifications:
- Experience working with hospice patients.
- Experience with electronic medical records.
- Bilingual in English and Spanish.
Responsibilities:
- Provide personal care to patients in their homes, including bathing, dressing, and grooming.
- Assist patients with mobility and transfers.
- Monitor and record vital signs and other health-related information.
- Communicate with the interdisciplinary team regarding patient care.
- Provide emotional support to patients and their families.
Skills:
As a Hospice Home Health Aide – CNA, you will use your excellent communication and interpersonal skills to provide compassionate care to our patients. You will also use your knowledge of personal care techniques and medical terminology to assist patients with their daily needs. Your ability to work independently and as part of a team will be essential in ensuring that our patients receive the highest quality of care. Additionally, your experience with electronic medical records and bilingual skills will be beneficial in this role.
