Job Summary
The Clinical Supervisor will be responsible for coordinating and managing patient care, supervising clinical personnel, and ensuring the delivery of high-quality hospice care. This role works closely with the Clinical Director and Medical Director to maintain compliance with accepted professional standards and to support the interdisciplinary care team.
Key Responsibilities
- Receive and review patient referrals to determine hospice care needs.
- Assign appropriate personnel and conduct case reviews through home visits, conferences, and record assessments.
- Provide guidance to clinical staff, including goal setting and care plan development.
- Participate in interdisciplinary case conferences to coordinate patient care.
- Assist in the hiring, orientation, and education of clinical staff.
- Participate in quality improvement initiatives and ensure compliance with organizational policies.
- Maintain a strong understanding of hospice care philosophy, symptom management, and end-of-life issues.
- Engage in community outreach and promote Forest Park Hospice’s role in the healthcare system.
Qualifications
- Current licensure as a Registered Nurse (RN).
- BSN degree preferred, with graduation from an NLN accredited nursing program.
- At least 2 years of management or supervisory experience in a hospice or home care setting.
- Excellent judgment, communication, and leadership skills.
- Strong understanding of hospice philosophy and pain/symptom management.
- Licensed driver with an insured, reliable vehicle.
- Current CPR certification.
