Clinical Hospice Scheduler – LPN

About the Role:

We are seeking a highly organized and detail-oriented Clinical Hospice Scheduler – LPN to join our team. In this role, you will be responsible for scheduling patient visits and coordinating with our clinical team to ensure timely and efficient care delivery. You will play a critical role in ensuring that our patients receive the highest quality of care possible. As a Clinical Hospice Scheduler – LPN, you will be an integral part of our healthcare team, working closely with our nurses, physicians, and other healthcare professionals to provide exceptional care to our patients.

Minimum Qualifications:

  • Active LPN license in the state of Oregon
  • Minimum of 2 years of experience in a healthcare setting
  • Strong organizational and time-management skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Preferred Qualifications:

  • Experience working in hospice or palliative care
  • Familiarity with electronic medical records (EMR) systems
  • Bilingual in English and Spanish

Responsibilities:

  • Schedule patient visits and coordinate with clinical team to ensure timely and efficient care delivery
  • Communicate with patients and their families to ensure that their needs are being met
  • Maintain accurate and up-to-date patient records
  • Collaborate with healthcare professionals to develop and implement care plans
  • Provide exceptional customer service to patients and their families

Skills:

As a Clinical Hospice Scheduler – LPN, you will utilize your strong organizational and time-management skills to ensure that patient visits are scheduled efficiently and effectively. You will also use your excellent communication and interpersonal skills to communicate with patients and their families, as well as collaborate with healthcare professionals to develop and implement care plans. Additionally, your ability to work independently and as part of a team will be essential in providing exceptional care to our patients. Familiarity with electronic medical records (EMR) systems and fluency in Spanish are also beneficial skills for this role.
