Clinical Hospice Liaison

 

Position Summary

The Hospice Clinical Liaison is a marketing support role that facilitates communication of clinical care needs for new palliative and hospice patients and supports company growth by utilizing clinical skills and clinical knowledge to increase admissions. The Hospice Clinical Liaison supports marketing team relationships with referral partners in the professional medical community (hospitals, physician offices, assisted living, long term care facilities) by visiting accounts weekly, providing educational inservices, meeting with patients and families to explain hospice, and signing paperwork, make visits to Palliative Care patients and assist with additional clinical needs as assigned.

Job Duties

1. Assists team members in achieving monthly admission targets.

2. Supports marketing team relationships with accounts by visiting them as assigned.

3. Tactfully markets programs and services offered by Home Health and Hospice.

4. Performs informational visits to patients and families as well as conducts explanation of hospice and palliative care benefits and signing of consents.

5. Assists with transition of hospice patients from acute care setting to home setting.

6. Provides courtesy tuck-in visits to hospice referrals in home setting as requested.

7. Makes nursing visits to palliative care patients, as assigned.

8. Assists in transitioning palliative care patients to hospice, as appropriate.

9. Makes nursing visits to hospice patients, as assigned.

10. Supports initiatives that impact referral and admission development. Enhances sales efforts by providing input into development of new tools and messaging to external audiences.

11. Identifies and promotes educational opportunities relative to Home Health and Hospice Care; Assist and/or delivers educational presentations to referral partners.

12. Presents features and benefits of hospice services, highlighting differentiators, asking probing questions, listening to service issues, and handling objections.

13. Promotes customer service with timely call backs, listening and understanding, good communication and positive attitude.

14. Works and communicates effectively with the team, including: marketing team, managers, directors, support staff and clinical staff.

15. Responds promptly to communication with Supervisor and other team members.

16. Practices the utmost confidentiality in the handling of patient information and medical records.

17. Complies with all HIPPA required protections for patient information and medical records.
